You should not make medical decisions unilaterally based on the vitals you acquire in IntelliH but rather to discuss the relevance of the data with a health care provider before taking appropriate clinical action.  Your Care team will send you an invitation to participate in the IntelliH Remote Patient Monitoring program.  These vitals are uploaded to your care team's dedicated IntelliH portal in the cloud.  • Care Plan: Your care team will send you a daily care plan that informs you the vitals and the medications that you are required to take.  • Measure and send Vitals: Using the devices provided to you by your care team take your vitals.  The IntelliH patient App will allow you to conveniently engage with your Care team from the convenience of your home.  Ask your provider if they are using the IntelliH solution to take care of patients at home.  This app is designed to facilitate communication with your healthcare provider and help you track health data as part of a remote patient monitoring program.  • Always seek the advice of your physician or other qualified healthcare provider before making any medical decisions or if you have questions about your health or a medical condition.  It is NOT intended to provide medical advice, diagnosis, or treatment, and should NOT be used as a substitute for professional medical care.  IntelliH can only be used by you if your care provider has enabled you to use this and you are in their care.  You can inform your care team the medications that you have taken through a single tap.  Your care team can review your diet as necessary.  Your care team will also provide you with one or more Vital monitoring device(s).  • Your Care team will list the medications that you have to take.  Using the code you will be able to set your access credentials and use the IntelliH App, pair your devices to the App and then take your vitals.  • You can interact with your care teams via secure Text, Health tweets as well as high-definition Video.  Always consult your healthcare provider about the interpretation of your vital sign measurements and any necessary clinical actions.  Your provider can see the vitals you have taken and can engage with you when necessary.  • Track Activity - You can sync your activity data using Apple Health.  • Never disregard professional medical advice or delay seeking medical treatment because of information obtained through this app.  The app reads step count and walking distance from HealthKit to help track your daily activity levels.  IntelliH helps to manage your medications.  • If you think you may have a medical emergency, immediately call your doctor, call 911, or contact your local emergency services.  • This app is intended for use only under the supervision and direction of your enrolled healthcare provider.  • Track Diet – You can track your diet by taking a picture of your food plate and identify the items and portion size.  The App will remind you to take your vitals and medications.  IntelliH tracks compliance through reminders as well as alerts.  Do not use this app to make independent medical decisions.  You can review your vitals if you are logged in.  IntelliH is an invite only App.
